Bulk Purchase/Course Purchase Request Form

Please use this form to make a purchase for 2 or more course enrollments or if you wish to make a purchase for someone else. 

Please have ready the full name and email of the individuals you wish to enroll in the course.

Once we receive your request we will email you with an online invoice for the total cost of your purchase.

Once payment is received we will automatically enroll each person you indicated on your form. An email will be sent to each person to confirm enrollment. If you have questions, email training@csh.org
